WHY PLAYERS GET STUCK

Do not loot Isaac's Treasure before speaking to Isaac. You still receive the shoulder armor, but the quest never activates and its difficulty-based XP reward is lost. Enter the Cultist's Lair first and talk to Isaac before opening the stump.

WALKTHROUGH
  1. 1
    Find the Cultist's Lair
    • From the Foggy Bank Rift, backtrack south along the Burrwitch road and find the ruined house occupied by cultists.
    • Open the trapdoor in the house floor. It leads to the Cultist's Lair and is not marked on the world map.
    • Do not open the stump west of the rift yet; looting it now forfeits the quest XP.
  2. 2
Speak to Isaac at the back of the lair
    • Fight through the cultists to the altar at the back of the cellar.
    • Speak with the dying Isaac beside the altar and let him explain where he hid his valuables.
    • A Sacrifice activates after the conversation and the objective changes to finding Isaac's hidden stash.
  3. 3
Recover the stash from the riverbank stump
    • Return to the surface, then go west from the Foggy Bank Rift and follow the river around to the small projecting bank.
    • Hover the loose tree stump and open Isaac's Treasure.
    • The quest completes immediately with no turn-in. You receive XP and the guaranteed Isaac's Spaulders.
STILL LOST?

If you cannot find the entrance, backtrack south from the Foggy Bank Rift and inspect the floor of the cultist-occupied house. The stash is west of the rift in a stump on a small riverbank. If you opened it before speaking to Isaac, the quest cannot be reactivated on that difficulty.

Looting the stash first still grants the unique shoulder armor, but speaking to Isaac afterward does not start the quest, so you lose the XP reward (Normal 250 / Elite 4,500 / Ultimate 19,000). This does not affect the main story or unlock any follow-up quest.
